1. Permission to take photos or videos YES, I authorize Tappin Studio Photography to photograph and/or record the athlete during the agreed session or activity. NO, I do not authorize Tappin Studio Photography to photograph or record the athlete.

I understand that images published online can be seen, copied or shared by other people. I can ask that an image stop being used in the future, although it may not be possible to recover copies that other people have already shared.

Terms of this authorization

Scope. Each permission applies only as marked above. The permission to publish is optional and independent of the permission to take photos. If permission 1 is NO, no photos or videos of the athlete will be taken, and permission 2 does not apply.
Minor's privacy. Tappin Studio Photography will not publish the athlete's full name, date of birth, school, address, phone or other personal contact details together with the images. A first name, jersey number or team name may appear only if it is visible in the image itself or you approve it separately.
No compensation. This authorization is given voluntarily and without payment or compensation of any kind to the athlete or the guardian for the authorized use of the images.
Copyright. Tappin Studio Photography owns the copyright to the photos and videos it creates. Any rights of the client to use purchased images are defined in the Client Agreement and in the purchased package.
Release. To the extent permitted by law, I release Tappin Studio Photography and its owner from claims related to the use of the images within the scope I authorized in this form. This release does not cover uses outside that scope.
Duration and withdrawal. This authorization remains in effect until I withdraw it. I can withdraw or change it at any time by writing to info.tappinstudio@gmail.com. Withdrawal applies going forward: Tappin Studio Photography will stop new uses and remove the images from the channels it controls within a reasonable time, but it cannot recall materials already printed or distributed, or copies shared by third parties.
Your data. The information in this form is used only to record and manage this authorization and to contact you about it. It is not sold or shared for marketing. A record of the submission is kept with its date and time (UTC), IP address and browser information as proof of consent.
Electronic signature. I agree to sign electronically. Typing my full name below has the same validity as a handwritten signature under the U.S. federal E-SIGN Act and the Uniform Electronic Transactions Act (UETA) as adopted in Florida. I can request a paper copy by writing to info.tappinstudio@gmail.com.
